Patient Registration

Patients wishing to register should complete the form below. Please also bring with you some evidence of identity and proof of address to confirm you live in our practice area. It will assist with obtaining your medical record if you can provide your NHS number (on your medical card or available from your last GP) and previous doctor's details. Examples of evidence of identity and address include: Passport, Driving Licence or other photo ID, public utility bills, bank / building society statements, letters from a government agency or solicitor. Please ask at reception for details. If you do not have any of the above, please ask at reception. All new patients are also asked to complete a comprehensive health questionnaire. 

Your full medical records will be transferred from your last GP practice automatically once your registration is processed by NHS England. Your recent notes can often be transferred electronically, but your full records can take a few weeks to arrive. If you had repeat medication at your last surgery, a doctor here will probably need to see you before prescribing more. Please bring your repeat prescription form from your last surgery with you.  As there may be dealys (outside of our control) in obtaining your full medical reports it is often helpful if you can bring with you any hospital letters for reception staff to scan to the records to help our GPs whislt we await your medical records to arrive. 

We will offer new patients an initial nurse review as part of a 'New patient check'.

All patients are allocated a "named accountable GP" when they register with the surgery. At Boundary Court this is Dr Shah for every patient.  This does not affect your right to ask for an appointment with a GP of your choice (for example, you might want to see a GP of the same sex).
